WAIVER Y LIBERACIÓN DE RESPONSABILIDAD (ESPAÑOL)
Reconocimiento, Asunción de Riesgo y Liberación de Responsabilidad
Al registrarme y participar en este evento organizado por Nueva Jerusalen Church, llevado a cabo en The Collective, 12401 S. Post Oak Rd., Houston, TX 77045, reconozco y acepto voluntariamente lo siguiente:
1. Asunción de Riesgo
Entiendo que mi participación en el evento puede implicar riesgos inherentes, incluyendo, pero no limitados a, caídas, lesiones personales, enfermedades, daños a la propiedad u otros incidentes. Acepto participar bajo mi propia responsabilidad.
2. Liberación de Responsabilidad
En la máxima medida permitida por las leyes del Estado de Texas, libero, exonero y descargo de toda responsabilidad a Nueva Jerusalen Church, sus directores, líderes, voluntarios, representantes, afiliados y al lugar del evento, por cualquier reclamo, demanda, daño, pérdida o lesión (incluyendo lesiones personales, discapacidad o muerte), ya sea causada por negligencia o por cualquier otra causa, que surja de o esté relacionada con mi participación en el evento.
3. Indemnización
Acepto indemnizar, defender y mantener indemne a Nueva Jerusalen Church y a todas las partes relacionadas de cualquier reclamo, responsabilidad, costo o gasto (incluyendo honorarios legales) que surjan de mi participación en el evento.
4. Autorización de Uso de Imagen
Autorizo a Nueva Jerusalen Church a tomar fotografías, videos y/o grabaciones durante el evento y a utilizarlos para fines promocionales, publicitarios y ministeriales, sin compensación alguna.
5. Condición de Salud
Declaro que me encuentro en condiciones físicas adecuadas para participar y que no existe ninguna condición que me impida hacerlo de manera segura.
6. Consentimiento Informado
Declaro que soy mayor de edad y que he leído, entendido y aceptado este acuerdo en su totalidad de manera voluntaria.
(Si el participante es menor de edad, el padre, madre o tutor legal acepta estos términos en su nombre.)
Legislación aplicable:
Este acuerdo se regirá e interpretará conforme a las leyes del Estado de Texas.
LIABILITY WAIVER & RELEASE (ENGLISH)
Acknowledgment, Assumption of Risk, and Release of Liability
By registering for and participating in this event organized by Nueva Jerusalen Church, held at The Collective, 12401 S. Post Oak Rd., Houston, TX 77045, I voluntarily acknowledge and agree to the following:
1. Assumption of Risk
I understand that participation in this event involves inherent risks, including but not limited to slips, falls, personal injury, illness, property damage, or other incidents. I voluntarily assume all risks associated with my participation.
2. Release of Liability
To the fullest extent permitted by the laws of the State of Texas, I hereby release, waive, and discharge Nueva Jerusalen Church, its directors, leaders, volunteers, representatives, affiliates, and the event venue from any and all claims, demands, damages, losses, or injuries (including personal injury, disability, or death), whether caused by negligence or otherwise, arising out of or related to my participation in the event.
3. Indemnification
I agree to indemnify, defend, and hold harmless Nueva Jerusalen Church and all related parties from any claims, liabilities, costs, or expenses (including attorney’s fees) arising from my participation.
4. Media Release
I grant permission to Nueva Jerusalen Church to photograph, record, and use my image, likeness, or voice for promotional, advertising, and ministry purposes without compensation.
5. Health Condition
I certify that I am physically able to participate in the event and have no condition that would prevent my safe participation.
6. Informed Consent
I confirm that I am at least 18 years of age and have read, understood, and voluntarily agree to all terms of this agreement.
(If the participant is a minor, a parent or legal guardian must agree to these terms on their behalf.)
Governing Law:
This agreement shall be governed by and interpreted in accordance with the laws of the State of Texas.
